What is the Topaz Procedure?
The Topaz Procedure (or Topaz MicroDebrider) is a minimally invasive technique used to treat chronic Achilles tendonitis and chronic plantar fasciitis. Topaz is designed for patients who have not achieved satisfactory symptom relief through more conservative treatments, such as orthotics, physical therapy, stretching, massage, or injections.
What you gain from Dr. Baravarian’s years of experience
While other practices promote Topaz as “the latest and greatest,” we’ve been researching and assisting with its development for over ten years. Consequently, Dr. Bob has extensive experience achieving the best outcomes for his patients using Topaz technology. He has also participated in several advanced research projects employing the Topaz probe for various foot and ankle conditions. It is a very exciting technology.
How does the Topaz coblation procedure work?
The entire process takes only 20 minutes and is conducted in a hospital under intravenous sedation (you are asleep but breathing on your own). Your doctor inserts the Topaz wand, which resembles a sewing needle, into the affected fascia or tendon for just half a second. Then, the wand is moved about a quarter-inch and inserted again. This motion is repeated until a grid-like pattern is established around the affected area.
The Topaz wand employs cobalation technologies, which are radio frequency waves that gently create microscopic “trauma” to the scar tissue your body hasn’t yet been able to heal. The procedure brings blood back to the affected area and restarts the healing process by increasing the growth factors in the damaged tissue region. The strategically placed Topaz wand significantly minimizes any damage to the surrounding healthy tissue.
Because the method is minimally invasive, recovery time from the Topaz procedure can be measured in weeks rather than months. After one week in a walking boot, most patients can start wearing athletic shoes with orthotic inserts.
What is the success rate of the Topaz procedure?
The topaz treatment is successful 80% of the time. It’s important to note that pain relief is not instantaneous, and it may actually worsen before it improves. However, once your recovery is complete, your pain is alleviated, and the tendon returns to normal. As long as you stretch the tendon and wear supportive shoes and/or orthotics, your condition should not recur.
Does insurance cover the Topaz procedure?
Most insurance plans cover the Topaz procedure. Dr. Bob has provided it in his practice and is happy to assist you if you need this procedure. Please note that he conducted the initial clinical studies on the Topaz procedure and has extensive experience with it.

Topaz procedure FAQs
Is the topaz procedure a radiofrequency treatment?
The Topaz surgical procedure is a minimally invasive technique that utilizes radiofrequency energy. During the Topaz Procedure, this energy is directed at the affected tendon or tissue to stimulate a healing response and encourage tissue regeneration.
Can the Topaz procedure help relieve heel pain?
This medical procedure is particularly well-suited for patients experiencing the discomfort of plantar fasciitis. Plantar fasciitis is characterized by the inflammation of the plantar fascia, a ligament located along the underside of the foot, extending from the heel to the toes. While plantar fasciitis is the most common cause of heel pain, it is not the only one. It’s important to consult a foot and ankle specialist (DPM) for an accurate diagnosis and appropriate treatment.
